The pharmacy processes data in order to provide good pharmaceutical care to clients. In doing so, we process privacy-sensitive information, also known as personal data. In some cases we work together with other organizations or companies. Personal data are therefore carefully processed and protected by us. We comply with the requirements of the privacy legislation. That means, for example, that we:

  • clearly state for what purposes we process personal data. We do so through this privacy notice;
  • restrict the collection of personal data to only those personal data necessary for the purposes for which they are processed;
  • first ask for explicit consent to process your personal data in cases where your consent is required;
  • do not disclose your data to third parties, unless required to provide the service requested or when we are required to do so by law;
  • when we share your data, make arrangements with third parties to.

Use of personal data

Purposes
In order to provide good pharmaceutical care, we collect personal data from you. This is necessary for providing medication, contacting you, and billing. Because we do not provide all services ourselves, but use other service providers for some services, it is sometimes necessary to share data. For example, if you pick up medication from a service pharmacy, we will share your medication records. We do this only with your permission.

Cooperation between pharmacies

Apotheek Lindenholt in Nijmegen, Apotheek Benraad in Terborg and Apotheek Ulft in Ulft work closely together in providing pharmaceutical care. To ensure the continuity, quality and safety of care, these pharmacies use shared work processes and supporting systems.

As a result, pharmacists and pharmacy assistants from one of these collaborating pharmacies may have access to your personal information, medication information and other information necessary for the provision of pharmaceutical care. They may access, process and supplement this data to the extent necessary for:

  • prescription processing;
  • medication monitoring;
  • answering questions about your medication;
  • carrying out pharmaceutical checks;
  • providing medications and devices;
  • the continuity of pharmaceutical care.

Only employees who need this data for their work will have access to your record. All employees are bound by their legal duty of confidentiality and professional secrecy.

Who does this privacy statement apply to?

This privacy statement applies to all of our clients. In addition, this privacy statement also applies if you visit our website, are a former client, want to become a client or wanted to become a client, but ultimately did not, and we are acting for your pharmacy.

Personal Data

Depending on the services you use with us, we may process the following personal data about you:

  • NAW data
  • date of birth
  • BSN
  • age
  • phone number
  • email address
  • sex
  • medical information
  • name of your health insurance company
  • IBAN
  • information, which you enter yourself, for example in the contact form
  • log-in information.

For your and our safety, camera images of the waiting room are recorded. These images are deleted after 3 weeks.

Consent

We are not allowed to process personal data about you just like that. We need a legitimate reason to do so. Often this stems from the treatment agreement or a legal obligation. Sometimes it is based on your express consent.

We may ask you for consent for several things. We ask for your permission to share your information with other health care providers. We also need your permission if you have your medication (data) collected by another person (for example, a family member or caregiver). You must give permission before we share data with a service pharmacy so that you can pick up medication there.

For children under 12, we ask permission from one of the parents. For children 12 to 16 years old, we ask permission from the child together with one of the parents.

You can withdraw your consent at any time. If you do not agree, or withdraw your consent, we will not be able to serve you properly. Appropriate care can only be provided if all relevant data is available. Moreover, we are obliged to create a medication record to guarantee the quality of (future) care.

You can give or withdraw consent to the provision of data in the data exchange system LSP (National Switch Point) to various parties at www.volgjezorg.nl.

Provision to third parties

Within the cooperation between Apotheek Lindenholt, Apotheek Benraad and Apotheek Ulft, authorized employees of these pharmacies may process personal data for the performance of pharmaceutical care. This processing takes place within the collaboration and is not considered as provision to external third parties.

In addition, we may provide data to other health care providers, such as a service pharmacy, hospital, general practitioner or acting physician, to the extent permitted by law or with your consent. We share this data only if you have given permission to share the data with the relevant party.

It may also be necessary to share data with third parties, such as a supplier or an administrative office. When we provide your data to a third party, we ensure (with an agreement) that your data is not used for other purposes. We also agree herein that your information will be deleted as soon as it is no longer needed.

Furthermore, we will not provide the information you provide to other parties, unless required or permitted by law.

Security and retention

Healthcare providers have a duty of professional secrecy and may not, as a basic matter, disclose anything to others. All our employees who process client data or learn about it have committed to confidentiality.

We take appropriate security measures to limit misuse of and unauthorized access to your personal data. For example, we ensure that only the necessary persons have access to the data, that access to the data is protected and that our security measures are checked regularly. Through logging, we keep track of who has accessed your data. This logging is kept for at least 5 years.

Healthcare providers are required to keep medical records, including medication records, for 20 years. Once the legal retention period has expired and you are no longer a client with us, the records are destroyed. If you remain a client, we will retain the personal data as part of future medication expenses.
