Electronic medical record sharing
Your primary care physician and pharmacy each keep their own records about you. This contains information that is important for your treatment. Your family doctor records what your symptoms are and what treatments you receive. Your pharmacist records which medications you receive and to which medications you are allergic.
These data can be exchanged with the LSP (National Switch Point).

Giving permission
Does your general practitioner or pharmacy use the LSP? Then he or she will ask you for permission to make your data available through this network. After your consent, your healthcare provider will register your data with the LSP by providing your citizen service number (BSN). -
Request data
Another health care provider can then request your data if it is necessary for your treatment. Using your Citizen Service Number, the LSP searches which healthcare providers have made data available about you. If data are available, the healthcare provider can request and view them.
The network only lists your BSN and which GP and pharmacy have data about you available. So your medical data is not stored in the LSP.

Regional
The LSP is divided into about forty regions. All GP surgeries, general practitioners and pharmacies, which use the LSP, are assigned to one of these regions and can join another region if necessary. Within such a region, healthcare providers exchange data with each other. An exception has been made for hospitals. They often have patients who come from multiple regions and can therefore exchange data with multiple regions.In principle, patients do not notice the region classification, unless they also visit healthcare providers from other regions. In that case, data may not be retrievable. Through the you can see in which region or regions your healthcare provider can exchange data.
Do you want health care providers to be able to share your medical information electronically? Then you can give verbal permission for this if your doctor or pharmacist requests it. In addition, you can print out the form, fill it out and submit it to your health care provider. You can also arrange your consent online at www.volgjezorg.nl. Multiple health care providers may have information about you that they want to share. If so, please give your consent with each healthcare provider. You can withdraw your consent at any time.
