Actual medication review
We make a review of all the medications you are currently taking. We also note which medications have been stopped or changed during your admission and why. This way you avoid accidentally continuing to take a medication that was stopped because of side effects. -
Informative talk
On or immediately after your discharge from the hospital, we will have a talk with you (and/or your caregiver). We will explain which medications you are taking now, which have been stopped, and which you will need in the future. This makes sure you know exactly what to do and helps avoid mistakes. -
Digital record
We put all the important data in your digital patient record according to the current guidelines. We also discuss this with you (and/or your caregiver) to make sure everything is correct. This is especially helpful if you take multiple medications. -
Transfer to GP
We share your medication history with your GP. This way he/she also knows which medications you are taking and which have been stopped. This prevents you from accidentally getting a medication again that you no longer need. -
Timely counseling
We do this counseling before or at the time your supply of discharge medication from the hospital runs out. This way we make sure you don't run out of medication and that care continues properly.

When may the pharmacy bill for 'this act'?
The pharmacist may charge for pharmaceutical counseling during hospitalization if all important data are in the digital patient record according to the current guideline 'Guideline Transfer of Medication Data in the Chain'. These data must also have been discussed with the patient or their caregiver, if necessary.
