Counseling and improving your medication use in collaboration with your primary care physician

From the pharmacy, we are happy to help you use your medications correctly. We do this in cooperation with your general practitioner. The goal is to make your medication use as safe, effective and appropriate to your health situation as possible.

How does this counseling and improvement of your medication use work?

We will review your current medication history with you. Here, we pay attention to how you use your medications, whether adjustments may be needed, and whether everything is well-matched. If we notice anything that can be improved, we will consult your doctor.

What can you expect?

  • You will receive counseling that suits your personal situation.
  • You will hear in advance what this counseling entails and what costs are associated with it.
  • You will give your own consent before we start counseling you more intensively.

Agreement with your health insurance

We have made agreements with your health insurance company. These include:

  1. What patient groups this guidance applies to.

  2. What steps we are taking to carry out this care well.

  3. How often this care may be declared, such as once a year.

  4. A further evaluation of medication may be desired in case of:

    • a request from the patient or informal caregiver;
    • signals of (potential) pharmacotherapy-related problems (FTPs) from GP or pharmacist or from third parties (such as hospital or home care), such as a recent fall from unknown cause, cognitive decline or reduced adherence;
    • an acute or structural change in health status.
