If you or a loved one is affected by dementia, we help you or your loved one use medication safely and effectively. We also support family caregivers in dealing with the care of a loved one with dementia. We strive to provide the best pharmaceutical care possible, focusing on quality of life. Together with other health care providers, such as the general practitioner, primary care assistant, geriatrician and district nursing, we work to improve the quality of life for people with dementia.

What does the pharmacy do?

We work according to a special care program that focuses on:

  1. Collaboration with health care providers

    • We consult with primary care physicians and other health care providers about how best to help you.
    • We share important information, such as the latest on medications and tests, to coordinate care.
  2. Recognizing Dementia

    • In the pharmacy, we pay attention to signs of dementia so we can discuss it in a timely manner.
    • Our team receives special training to better support you.
  3. Mapping your needs

    • Together we look at what symptoms and needs you and your family caregiver have.
    • We use questionnaires to discuss your medication use, possible side effects and your self-efficacy.
  4. Adjust and improve medication

    • We look at whether your medications are working well and whether they are still needed. That way we can stop unnecessary medications.
    • We help you take your medications safely, properly and on time.
  5. Follow-up care and support

    • We keep in touch regularly to check on how you are doing.
    • You can always come to us with questions or problems about your medications.
    • If necessary, we will also come to your home or consult with other caregivers.
