Day of the Practice Support Person (POH): local cooperation is vital for your health
Today we celebrate Practice Support Person (POH) Day. On this day we put all practice support workers in the spotlight. We want to thank them for the nice cooperation.
Because that local cooperation is crucial for your health and medication safety!
Who is "the practice support person?"
The practice support person (POH) works in general practice. They help guide people with chronic diseases such as asthma, COPD, diabetes or cardiovascular disease. You can also go to the POH for help with quitting smoking or for psychological complaints (thuisarts.nl).
Collaboration between pharmacy and POH
As a pharmacy, we work closely with your primary care physician and the practice manager. By working well together, we make sure you get the right care and guidance. For example, when adjusting medication for diabetes or monitoring inhalation use for asthma. The POH checks with you how you are doing and from the pharmacy we support you with advice on medicines and resources.
.Why is good cooperation important?
Good cooperation means better care for you. If the POH notices something about your medication use, together we can quickly discuss adjustments with the GP. So you don't have to tell your story over and over again. We also help the POH with information about new medications or inhalation aids.
Fine cooperation, thanks to you
We would like to thank all local practice supporters for the pleasant cooperation! Thanks in part to them, we can help you even better from the pharmacy. For example, do you need help with taking your medication correctly? Then we work together to make a good plan. We also make sure that important check-ups, such as blood pressure readings or lung tests, are done on time.

You may not always notice it right away, but behind the scenes we consult regularly. For example:
- Discussing use of medication for diabetes or asthma
- Attention to side effects of medication
- Together determine whether your medication needs to be adjusted
- Tips and guidance for smoking cessation
